A Fumigation Management Plan is written before a phosphine fumigation starts. It records the decisions and confirmations your product’s labeling asks you to make, and it stays with the job records afterwards.
The container label and the applicator’s manual for the product you use govern the work. These templates do not replace them, they are not legal advice, and completing one does not by itself establish compliance. The fumigator in charge and the site representative work through the plan together, and both sign it.
Pick the packet that matches the product, the site and the pest you are treating. If none of them matches, the packet is the wrong tool for that job and adapting it is not the answer.

Use one label-first FMP for outdoor burrowing-pest treatments. Proceed only when the exact current labeling for the phosphine product expressly permits the product form, target pest, site and burrow use.
The current label and applicator’s manual govern. Federal, state, tribal, county and local requirements may add conditions or be more restrictive.

Complete this form with the current container label and applicator’s manual for the exact product and EPA registration number in hand. Record every applicable label requirement before treatment. If the labeling does not expressly permit the job, do not treat.
For complex sites, attach a separately identified sketch when the form’s sketch area is not large enough.
Six record sheets used with either plan: monitoring and clearance, personnel, application quantity, sealing and placards, entry, and a residue sheet you complete only if spent product, dust or a spill was retrieved. Readings, crew, quantities, seals and entries are not written into the plan itself — the plan points here, and these sheets are where the running record lives.
Print the sheets your job needs, number them, and file them with the completed plan.

The labeling governs. Where a packet and your labeling differ, follow the labeling and tell us so the packet can be corrected. Nothing here is legal advice, and nothing here is a regulatory, government or manufacturer endorsement.
Worker Protection Standard. Stored commodities are excluded from the Worker Protection Standard by 40 CFR 170.303(b)(5) and (b)(8), so the stored-grain packet contains no WPS elements. For burrow work the Standard reaches only vertebrate control directly related to producing an agricultural plant on an agricultural establishment.
OSHA. Each employer decides which standards reach its own employees; these packets make no determination. Federal OSHA applies only the part 1910 standards listed in 29 CFR 1928.21(a) to agricultural operations. 29 CFR 1910.146 does not apply to agriculture. 29 CFR 1910.272 reaches the grain-handling facilities it lists, not on-farm storage.
Exposure readings. Where 29 CFR 1910.1000 applies, OSHA Table Z-1 lists phosphine at 0.3 ppm (0.4 mg/m³) as an 8-hour time-weighted average from breathing-zone samples — a single reading is not that average.
